Background sync for Nightscout Local and Dexcom Share
Nightscout Local and Dexcom Share never got the nudge that keeps phone connections syncing in the background, so on iPhone they only synced with the app open. They now get it.

No background sync on iPhone: Every five minutes we send phones with an on-device connection a silent nudge to sync. Nightscout Local and Dexcom Share were missing from that list. On iPhone the nudge is the only way to sync in the background, so these two only synced with the app open. On Android it is the backup for when the system pauses syncing. Both connections now get it.
Phones dropped from background sync: One unusable push token in a batch could stop other people's phones receiving the nudge from then on. Each phone is now handled on its own.
Sleep sessions lost: When one sleep session in a delivery was skipped, the sessions after it were lost too. Now only that one is skipped.

Nudges where they are needed: Phones that have just synced are skipped, so the nudge goes to the ones that have gone quiet.
